- Filter > Stylize > Glowing Edges.

- Filter > Blur > Motion Blur.

- Edit > Transform > Perspective.

- Add Vector Mask. Fill it with linear white/black gradient, just like in example below:

- Create a new layer. Fill it with linear black/transparent gradient. Multiply Blending Mode, Opacity 58%.
- Erase the top central part using soft round brush or Eraser Tool.
